Insurance:
PPO:
We accept most PPO's including Blue Shield, Anthem Blue Cross, Aetna, Cigna, PacifiCare, United Healthcare, Health Net, and Medicare.
HMO:
The only HMO we accept is John Muir Health Plan. Please obtain a referral from your plan if you want to see me. If we do not take your HMO (ie Kaiser, Hills, Brown and Toland, Alta Bates, East County) and you cannot get an out of network referral, then I can see you under a cash basis as below.

Cash Policy for Needle Aponeurotomy:

A consultation fee of $125 is charged if no procedure is performed that day.

NA on one finger is $570, and additional fingers on the same day are $285 each.

I do not perform the NA on both hands the same day.

Fees are paid on the day of service by cash, credit card, or debit card.

There can be other charges if Xrays, injections, or other procedures are needed.
